Throughout these Terms, “SportMed Connect,” “we,” “us,” or “our” refers to SportMed
Connect PLLC, Direct Access Medical LLC, and their affiliated entities.

These Terms govern your access to and use of the SportMed Connect website, digital platforms, and any information or services made available through them. These Terms are incorporated by reference into any subscription agreement, master services agreement, order form, or similar contract unless expressly stated otherwise.
2. Authorized Acceptance
These Terms may be accepted by:
• an individual enrolling themselves;
• a parent or legal guardian enrolling a minor;
• an organization, including clubs, facilities, or school districts, enrolling participants.
By accepting these Terms, you represent that you have legal authority to do so.
3. Modification of Terms
We may modify these Terms at any time. Updates are effective upon posting.
Continued use constitutes acceptance.
4. Informational and Educational Use Only
Content provided through the website is for informational and educational purposes only
and is not a substitute for professional medical advice, diagnosis, or treatment.
5. Telehealth Consent
You consent to the use of telehealth technologies. You understand that telehealth has
limitations, is not appropriate for emergencies, and may require referral to in-person care.
6. No Emergency Services
SportMed Connect does not provide emergency medical services.
Call 911 in the event of an emergency.
7. No Guarantee of Outcomes or Return-to-Play
SportMed Connect makes no guarantees regarding outcomes, recovery timelines,
return-to-play decisions, or eligibility determinations.
8. Minors and Parental Consent
Parents or legal guardians enrolling minors represent they have authority to consent and
are responsible for the minor’s participation.
9. School Districts and Educational Institutions
Schools represent that required parental consents have been obtained. SportMed Connect
does not act as an educational provider and does not assume FERPA obligations.
10. Health and Education Records
SportMed Connect maintains healthcare records subject to HIPAA
where applicable and isnot responsible for FERPA compliance.
11. Reliance on User-Provided Information
We rely on information provided by parents, schools, and participants and are not
responsible for outcomes resulting from inaccurate or incomplete information.
12. Technology Limitations
We are not responsible for connectivity issues, interruptions, or third-party technology failures.
13. Limitation of Liability
To the maximum extent permitted by law, SportMed Connect shall not be liable for indirect,
incidental, consequential, or punitive damages. Your sole remedy is to stop using the services.
14. Indemnification
You agree to indemnify and hold harmless SportMed Connect from claims arising out of
your use of the services or violation of these Terms.
15. Intellectual Property
All content is the property of SportMed Connect and may not be used without prior written consent.
16. Termination
We may suspend or terminate access at any time at our discretion.
17. Governing Law and Venue
These Terms are governed by Texas law. Venue is Dallas County, Texas.
18. Entire Agreement
These Terms constitute the entire agreement regarding use of the website and services.
